This form serves as your application for one of thirteen $1,500 scholarships aimed at aiding students who have completed the Miami-Dade Juvenile Services Department (JSD) Diversion or Prevention Program in pursuing higher education. Eligibility Requirements:
Individuals must be between the ages of 16-20 years:

Completion of a JSD Diversion or Prevention program within 1 year of application
High school senior or recent graduate from an accredited high school or GED Program- official transcript or copy of diploma required
